Welcome to our Patient Portal.
We are now using electronic records and have made it easier for you to enter your
demographics and medical history prior to your appointment.
You will need the security code in the e-mail we sent you to do this.
Rest assured that all information provided is confidential, secure and HIPAA compliant.
IMPORTANT NOTE:
Please..only click finalize after you are sure you are done completing all of your information.
Once you click Finalize you will no longer be able to access or change your form.
Please click save after completing each page of information.
Here are the simple steps to enter your information.
STEP 1: Click on this secure link ~ PATIENT PORTAL ~ to access our secure site to enter your demographics and medical history. |

STEP 2: Below the username and password is a link that says, "Don't have a username for your account? Use your Security Code to create one now." Click on this link to create your own unique username and password. Your security code is located in the e-mail we sent you. |

STEP 3: Once you create a username and password, you will have access to your information. The first window you will see will have these links:
Click on Demographics to get started. |

STEP 4: Once you feel this section is complete, just click "save". Doing this will automatically bring you to the next topic : Insurance. Once you feel this section is complete, just click "save". TIP: You may also move from topic to topic by simply clicking on the topic link, but remember to click "save" first, to save the information you have entered. |

STEP 5: Next you will arrive at the Forms topic, Just fill in the information and click "next " or "previous" to navigate through the sections of the form. WARNING!! Please be sure you are completely done filling out your form before you click the finalize button. When you click finalize, you will no longer be able to fill in or change any information you have entered. If you are not finished and want to come back to this form, just hit save and log out. Once you have completed all of this information, click " finalize " at the bottom of the screen. |

STEP 6: Now all that is left to do is to click "log out" to exit your web session. That's all there is to it. |
