Our Office
Our office is located in Scottsdale, Arizona. We moved August 1, 2004 our address is:

You may reach us by telephone at (480) 657-7006, or toll-free at (866) 876-6329. We are open Monday through Thursday, 9:00 a.m.–5:00 p.m. with a lunch break 12:30–2:30. We are not available for general-information calls on Mondays.
While you are in Scottsdale, we provide a patient transportation service, to save you from the hassles of driving around a foreign place and trying to find our office and the hospital. This is a service we provide to our patients as a courtesy, and if you request it we will pick you up from the airport, take you to our office for your pre-operative appointment or consultation, bring you to the hospital for surgery and pick you up afterwards, and take you to the airport when you leave Scottsdale.
You may plan to drive while in Scottsdale, but we will still transport you to the hospital prior to your surgery if you are traveling alone. However, please note that transportation is limited to patients staying in the downtown Scottsdale area.
Driving to our office:
- From the north: Ariz. Loop 101 south to Indian Bend Rd. (exit 44). Right at stoplight onto E. Indian Bend Rd. Continue to N. Scottsdale Rd., a major intersection, and turn right. Turn right again past the shopping complex. We are located in the three-story building with the Waste Management sign.
- From the south: Ariz. Loop 101 north to Indian Bend Rd. (exit 44). Turn left off of ramp onto E. Indian Bend Rd. We are located immediately to the right, near the McKesson sign. Continue to N. Scottsdale Rd., a major intersection, and turn right. Turn right again past the shopping complex. We are located in the three-story building with the Waste Management sign.
- From the west: (South) E. Lincoln Dr. or E. Indian School Rd. east to Scottsdale Rd. Turn left onto N. Scottsdale Rd., and continue just past E. Indian Bend Rd. Turn right after the shopping complex. We are located in the three-story building with the Waste Management sign. (North) E. Shea Blvd. east to Scottsdale Rd. Turn right onto N. Scottsdale Rd., and continue just past E. Indian Bend Rd. Turn left after the Radisson Hotel. We are located in the three-story building with the Waste Management sign.
- From the east: (South) E. Indian Bend Rd. west to Scottsdale Rd. Turn right onto N. Scottsdale Rd., and continue just past E. Indian Bend Rd. Turn right after the shopping complex. We are located in the three-story building with the Waste Management sign. (North) E. Shea Blvd. west to N. Scottsdale Rd. Turn left onto N. Scottsdale Rd., and continue just past E. Indian Bend Rd. Turn left after the Radisson Hotel. We are located in the three-story building with the Waste Management sign.
