Dr. Meltzer is board certified in Plastic and Reconstructive Surgery. He has been in
practice since 1990. He started at Oregon Health Sciences University and went into
private practice in October of 1996. He does a full range of
cosmetic plastic and reconstructive procedures.

| Dr. Meltzer practices at the Greenbaum Surgery Center in Old Town Scottsdale, Arizona,a state-of-the-art facility that has a level one trauma center and is accredited by JCAHO. |
| Dr. Meltzer works with Dr. Burt Webb (Gynecologist) for hysterectomy, oophorectomy and vaginectomy and other gynecological procedures and Dr. Mitchell Kaye (Urologist) for complex urethral procedures and repairs. |
| Dr. Meltzer adheres to the same guidelines and principles of WPATH (formerly known as HBIGDA). |
| Prior to any surgical procedure, prospective patients must fulfill the criteria established by WPATH Standards of Care and have letters of referral from a therapist (psychiatrist, psychologist or psychiatric social worker) who has had a long-term relationship with the patient. Your therapist should have a copy of these standardsor we can send you a copy. |
