Fees
The fee for a consultation is $100.00. We do ask for a credit card over the phone to hold a consultation date. If you need to cancel or reschedule just give us 48 hours notice and we will refund your card or make another appointment. It is recommended that you call about two to three months ahead of time for a consultation.

Additional fees incurred may include but are not limited to the following: transportation, medications,
meals, lodging, ancillary services and return to the operating room.

Payment
All patients are required to submit a $1000.00 non-refundable deposit to schedule a surgery.
A $2500.00 non-refundable deposit is required for any surgery over 4 hours.
If you need to cancel or reschedule your surgery date, you will need to give at least eight weeks' notice.
Your deposit will reserve your surgery date. Every attempt will be made to honor your date,
but circumstances beyond Dr. Meltzer's control may necessitate a date change.
50% of fees are due six weeks prior to surgery and the remaining 50% are due four weeks prior to your surgery.
You can pay for your surgery by cashier's check, money order, credit card (we take all major credit cards),
or cash. Some patients find it convenient to have their bank wire the funds.
Sorry, we cannot accept personal checks.
Insurance Policy
Dr. Meltzer does not participate with any insurance plans.
Dr. Meltzer does not participate with any health insurance companies.
Our office will not be in your network but Greenbaum may be and it may be listed under Scottsdale Healthcare Osborn. Some patients have been able to get portions of their surgeries covered by insurance but this is
something that you will need to discuss with your insurance and research on your own.
We are very happy to help you with insurance questions and concerns but it is a very complex situation and it is best that you call the office with any further questions. The patient will need to pay the fees in advance and submit their own claim. Dr. Meltzer will provide you with an itemized statement to submit to your insurance company.
Financing
We do not offer financing through our office but we are currently working with Medical Fianancing.com.
